On the next few pages you will be asked
to fill out several forms.
If you are a single applicant, you will need to fill out one
form with your personal information (Name, Age, Date of Birth,
etc) and one form with mailing information (the physical address
where your winning notification will be sent).
The address you provide has to be valid for at least another
year. The address that you provide can be yours or your parents',
friend's, etc.
If you are a family applicant, you will have to fill out a
separate form with personal information for each dependent.
You will only have to fill out one form with your mailing
information.
After you submit your information, you will be able to Log
In and track the progress of your application. Please use
the Client Login link on the control menu panel.
Before you start, please have the following ready:
Personal information for you and your family (if family
applicant)
The address where you would like the winning notification
to be sent
Scanned photos of you and, if applicable, of your family.
Every person needs to be photographed separately, looking
straight into the camera, with light plain background.
We will resize and format your pictures to suit the standards.
We will contact you if you need to resubmit your picture.
